REQUIRED Medical Permission Form

Please print, fill out, sign, and send this form to WHBA. This needs to be completed and sent to WHBA in addition to the online registration form and payment. 

To return completed form electronically, email file to GOwhba@gmail.com. Paper forms may be handed in to either school's main office, or hand it in person to one of our co-commissioners, Josiah Brown or Millie VandenBroek.
